Pickup and Delivery Option through Extra Product Option Plugin on Product

Hi there,

I want to display product delivery option on Extra Product Template for example:

If customer see the detail of the product the customer should have the two option in radio button ( Pickup OR Delivery).
If customer select Pickup radio box option then some text fields should appears below it and asking to fill some details(Name, Pickup date and Time and text area to write some extra details) same go for Delivery option as well.

I think you understood what i am trying to say at above. Please guide me over thing this urgent to do.
