Double radio buttons, Extra Product Options

I have this strange thing of having two radio buttons slightly on top of each other (check image). Other one is basic rectangle checkbox and the one below is the right format round radio button.

Any idea what could cause this and how to get rid of the one on top? I could propably css display:none the top button, but would be more convenient to have and option to check on/off in settings…
