Problems with extra products options and quantity box button

Hello, I have a problem with EXTRA PRODUCTS fields. Site show in the same line as Quantity box, not before that box.
And style for boxes is different as your example…
I show you a screen capture…

How to fix the extra products options??? Probably it’s a setting in that plugin, but I was checking and didn’t find anything about it…
